  • To Prepare for Your Appointment

  • Below are some helpful tips to prepare you for your appointment. Please register on our online patient portal prior to your visit, Patient Portal Registration.

    1. Please arrive 15 minutes prior to your appointment. We value your time and want to keep you on schedule.
    2. Please bring your child’s insurance card, along with the required co-payment for your visit.
    3. Please complete a Patient Registration Form.
    4. Bring a list of your current prescriptions, including any over-the-counter medications that you are taking.
    5. Bring a list of allergies or any adverse reactions to mediations that may have occurred in the past.

  • Scheduling Appointments

  • When scheduling future appointments, be aware we open our calendar 12 months in advance. Please call 8-10 weeks prior to required annual physicals to secure a day and time that works best for you.

  • Adolescent Visits

  • CPM cherishes health care to all patients from birth through adolescents and until 21 years of age.

    We provide, Wellness Visits, Sports and Camp Yearly Physicals and Sick visits. Call our office today!

  • Allison Stowell, MS, RD, CDN
    The Center for Pediatric Medicine is pleased to offer in house nutrition counseling. Our Registered Dietitian, Allison J Stowell MS, RD, CDN, is available to meet with your child or family to discuss your nutrition concerns including over/underweight, allergies, eating disorders, disease prevention and more. Her balanced view on nutrition, which emphasizes a non-diet approach and increasing mindful eating as the key to making better food choices is ideal for helping young people grow to be healthful teens and adults.

  • Transferring to CPM

  • Transferring to CPM is very easy. Please provide the office with any record release information before arriving on your first visit. CPM welcomes new and transferring patients. Please contact one of our two office locations to schedule your first visit. Our staff will assist you on how to have your records transferred and what to expect on your first visit.
